Forms<\/span><\/h1>\n

Although we do not transact business over the internet you are free to download or print any of these forms and then bring them or post them to the office for our volunteers to process. If you are not sure which form to use or are unable to print these forms, just give us a ring and we will be happy to assist.<\/p>\n

New Membership Form (Adult)
\n<\/a>To join DFCU you will need to complete this<\/a> form, deposit a minimum of \u00a35 cash or cheque and provide proof of Identity and also proof of address.
\nAny of the following are acceptable:
\nIDENTITY: Passport, Driving Licence, Dacorum Card, Notice of Tax Coding, HM Forces ID card, Birth Certificate.
\nADDRESS: Utility Bill, Bank Statement, DWP Letter, Council Tax Demand, two recent payslips.
\nPlease contact the office to arrange an appointment to open your account.<\/p>\n

Junior Member Application Form<\/a>
\nApplications must be accompanied by a copy of the Birth Certificate and \u00a35 cash. Junior accounts are operated by an adult until the member attains the age of 18 at which time they will be converted to an adult member.<\/p>\n

Savings (shares) Withdrawal Form<\/a>
\n(Please note that withdrawals take 30 days to process)<\/p>\n

Loan Application
\n<\/a>This form is now used for all loan applications including Speed or Secured loans. If your loan amount exceeds your share balance you will also need to complete an income and expenditure form (below).<\/p>\n

Income and Expenditure Form<\/a>
\nYou need to complete this form if your loan amount will be higher than your shares (savings) balance.<\/p>\n

Change of Details<\/a> (address, etc)<\/p>\n

Christmas\/Savings\u00a0Club Application<\/a>
\n(Use this form only for a Savings Account which locks away your savings until November).<\/p>\n

Standing Order Form<\/a>
\n(Use this to make regular payments from your bank account into your savings or loan accounts).<\/p>\n

Share Transfer Form<\/a>
